Good questions. The only movies I know where the director really goes out of his way to encourage, and wait for, real femal orgasms are exactly two: Ernest Greene (my husband), and Randy West (In his Real Female Masturbation series).
For other directors, a real female orgasm is icing on the cake, but not necessary for the scene to go well.
There are some actresses who try to hold back orgasms for their private lives, but they're not too common. Most don't have real orgams because the movies aren't set up for them. Usually there are no buzzing toys around, and they just don't have the time or inclination to wait for it.
Many women can't come on the set, for various reasons. They may like the action, and they may even get turned on, but they don't go all the way. Whether or not they masturbate when they get home depends on the woman. It's quite rare for a woman to have to masturbate right after a scene.

No director can demand real orgasms from women, and none do. Ernest encourages the women to do what they like, for as long as they like. We can always edit out the "slow" stuff later! Most don't care, or are happy if she fakes it well.
The women who come easily, have orgasms on camera. The ones who don't, like me, have them rarely. I've not talked to anyone about their philosophy regarding orgasms at work.
Percentage-wise, I'd conjecture that less than 15% of women have real orgasms at all. Five percent or so have them regularly, as they are women who come easily. Aria is in this catagory. Her orgasms are real, as she can have them several different ways.
As for me, personally, my philosophy has always been this: my orgasm is not the reason I'm on a set. So, I don't care if I have one or not, and I don't particularly try to have them. I don't particularly try not to, either, it's just not important. My kink is doing a scene, having the different partners, knowing that people are enjoying the show from their homes, putting on a good and believable show. I have always done things on camera that I do at home already, for free, so I'm always having a good time. In over four hundred tapes, close to a thousand scenes, there are only five or so that I really hated all the way through.
Remember, I'm a performer. I love what I do, and that what I do is sex, but the mission objective is to leave behind a good, hot, fun, timeless scene that will please viewers always. I can't let cramps get in the way of that, too hot a set, too cold, etc. As a professional, my job is to do the scene. Things that kill a day's work are things like out-and-out illness (fever, chills, pinkeye,coughs, etc.), injury, active STDs or a death in the family. Also these days, having an out of date test is grounds for being sent home.
The "frustrated actress" syndrome is extremely rare, as many of the women are not that engaged in the action to really get turned on. I don't want to ruin your fantasy, but a lot of the performers are very vanilla and "normal" at home. There is a small but steady segment of the female talent who really love the work on the sensual/sexual level, and the rest have made differing degrees of peace with their work.
